Step 1
Measure all female hormones-FSH, estradiol, progesterone and testosterone. This determines menopausal state and is used to determine modification of hormone dosages throughout therapy.
Step 2
Office visit for complete examination and other assessments: thyroid, blood sugar, mammogram, bone density, cholesterol.
Step 3
Record menopausal symptoms so these can be re-evaluated later to make further changes as needed.
Hormone dosage: Get patient understanding on method of administration. Start with lower dose of estrogen and testosterone which can be raised later depending on patient response. Progesterone must be included unless patient has had a hysterectomy, in order to prevent excess stimulation of the uterus. If any vaginal bleeding occurs, pelvic ultrasound is needed to evaluate the uterus. Bleeding is generally controlled by raising the progesterone dose or lowering the estrogen dose.
Hormone pellets are the strongest and most effective method of hormone replacement. See the patient for re-evaluation one month after starting therapy, thereafter in 3 months. Pellets should be reinserted every 3 months.
Hormone therapy is generally maintained for years, often indefinitely. If it is discontinued, the benefits will stop.
Step 1
Measure testosterone level in the blood.
Step 2
Do a full physical exam.
etc...
Then get other labs to include PSA for prostate evaluation, complete blood count and complete metabolic panel. Testosterone stimulates production of red blood cells, so blood count needs to be followed periodically. If blood count goes too high, phlebotomy is needed to lower it. The presence of prostate cancer is a contraindication to testosterone therapy unless the cancer has been treated or determined not to be aggressive.
Determine the best route of administration for the individual based on patient preference.
Start with lower dose of testosterone, which can be increased according to patient response.
Pellets are the strongest form of testosterone therapy. Anastrozole is also used to block conversion of testosterone to estrogen, and is included in pellet therapy. Pellets are replaced every 4 months. PSA and CBC are retested al least yearly.
Hormone therapy is generally maintained for years, often indefinitely. If it is discontinued, the benefits will stop.
We believe that the most important aspect of healing is the communication established between doctor and patient. Accordingly, we spend a lot of time talking to patients and their families, establishing rapport and positive energy. We do not believe that symptoms should lead automatically to medication, that people are natural repositories for drugs, or that a sick human being is just a machine in need of repair.
We believe that people are intelligent, spiritual beings who respond best to understanding and honest communication.
We believe that medicine needs to change so that all physicians address their patients as unique individuals, not just as a collection of illnesses.
As a new patient, you will have approximately one hour to spend with your doctor. During that time, your health history will be carefully reviewed, any necessary physical exams performed, indicated laboratory studies will be requested, and an initial treatment plan formulated.
Frequency of visits varies from patient to patient. We tailor this to meet your health care needs.
Patients often come back more frequently in the beginning, then less and less often as their health improves.
We have a large pediatric population in our practice.
We treat children for a variety of health concerns ranging from ear infections to ADHD.
We offer a variety of services treating a wide range of health concerns. Many of our patients have all their health needs met at our clinic.
We have many patients who receive ongoing care from their current primary care physician and visit our clinic for more specialized care, others have chosen one of our doctors as their primary care doctor.
